New Clients Instructions:

Please click on the corresponding “Counseling Forms” below, print the form, and bring the form completed to your first appointment
If you have never been to the office please add another 10-15 minutes to your estimated time needed to arrive on time to allow yourself time to get familiar with the office location.

Child/Teenager Forms – If applicable please make a copy we can keep of the most recent report card, notes from the school and/or psychological testing if applicable.
